Practice management for behavioural health

Built around the claim, not just the note.

CareOS runs a therapy practice end to end — scheduling, charting, a self-service client portal, and an insurance billing pipeline that files claims, watches for the acknowledgement, repairs what bounces, and reconciles the money that comes back.

Most therapy systems stop at producing a claim. CareOS is built around what happens after it is sent.

The gap

Submitting a claim and working a claim are different purchases

Practice software files the claim. Then a billing service — usually charging a percentage of everything the practice collects — chases the rejection, fixes the code, refiles it, and posts the payment when it finally lands. That second job is where the time and the money go, and it is the job CareOS was built to do.

1Scrub before it is sentCatch what a payer would reject while it can still be fixed for free.
2File and watchClaims go to the clearinghouse, and acknowledgements are polled and parsed — because an accepted file summary is not the same as an accepted claim.
3Repair and refileA correction queue, rather than a spreadsheet and somebody's memory.
4ReconcileElectronic remittance is read and matched against what was owed, so the ledger is one number and not an estimate.

What is in it

Everything a practice runs on, in one place

Revenue cycle

The largest part of the system.

  • Claim generation and scrubbing
  • Clearinghouse filing and acknowledgement tracking
  • Eligibility and benefits checks
  • Electronic remittance and reconciliation
  • Superbills, allowables, copay and deductible tracking

Scheduling

  • Calendar with per-provider booking pages
  • Confirmations and reminders
  • Telehealth links on the appointment
  • Shared calendars across staff
  • Service locations and availability

Clinical documentation

  • Session notes tied to the appointment
  • Co-signing for supervised clinicians
  • Group notes, written once per group
  • Note and form templates
  • Uncharted-work detection — sessions still missing a note

Client portal

Nearly as large as the billing engine, and deliberately so.

  • Self-registration and onboarding
  • Choose a therapist, book appointments
  • Forms and e-signature
  • Insurance entry and benefits verification
  • See a balance and pay it
  • Secure messaging

Money

  • Card payments, and public pay links with no login
  • Payment plans with reminders
  • Collections worklist
  • Refunds that unwind the way the payment came in
  • One ledger — one answer to what is owed

Practice operations

  • Dashboard and a Today view
  • Alerts and a shared to-do list
  • Groups and couples as first-class
  • Duplicate detection and client merge
  • Staff, providers, reports

Where it came from

Written inside a working practice

CareOS exists because a real therapy practice outgrew its EHR. It runs a live caseload every day — the scheduling, the notes, the claims and the money are not a demo. The parts that got built first are the parts that hurt most, which is a better specification than any feature list.

What it costs

Priced against the biller, not against the note-taking

An EHR subscription is $29–99 a month. The billing service beside it takes 6–8% of everything the practice collects. CareOS costs more than the first and replaces most of the second.

Solo · 1–2 clinicians $149 per clinician, per month

Everything, including the whole claims pipeline.

Group · 3–9 $129 per clinician, per month

Adds supervision and co-signing, groups and couples.

Scale · 10+ $109 per clinician, per month

Annual agreement, priority support.

Unlimited claims and remittances, at no extra charge.

Everyone else meters them. The amounts are small; being charged for the act of getting paid is the part practices resent.

  • No per-claim fee
  • No charge for electronic remittance
  • One ledger, so there is one answer to what a client owes

Twelve months for the price of ten if you pay annually. Onboarding — chart migration, payer enrolment, ERA setup — is quoted separately, because it runs on the payers’ timetable rather than ours.
